Learning outcomes

At the end of the module, the student possesses the knowledge relating to the methodology, organizational and clinical assistance models for taking charge, management and continuity of assistance in chronic situations. Develop knowledge related to therapeutic education. Knows the resources of the family and the community.

Course contents

Concepts of illness, chronicity, disability, comorbidity, fragile person.

• Chronic diseases and their characteristics - The complexity of care

PNRR and Decree 23 May 2022, n. 77 “Regulation defining models and standards for the development of territorial assistance in the National Health Service”.

• The multidimensional evaluation in chronic diseases and disabilities: - International Classification of Functions - ICF - The multidimensional evaluation scales

• Main problems and assistance in chronic situations:

- Fragile Elder

- Dementia - Alzheimer's disease

-Parkinson's disease

-Amyotrophic lateral sclerosis

-Multiple sclerosis

- Post-acute spinal cord injury

• Organizational models in chronic situations (chronic care model, self management, remote assistance and remote assistance)

• The network of services for the elderly and in the area

• Rehabilitation: accessibility, aids, assistance

• Trasitional care - from the hospital to the territory: the path of protected discharge and the activities of the Hospital-Territory Continuity Unit (NUCOT)

• The care giver.

• Therapeutic education: principles, aims, methodology, tools and evaluation. The motivational interview
