Learning outcomes

Upon completion of the module, the student possesses knowledge related to the methodology, organizational and clinical care models for taking care of the patient/family/community for the design and management of care in chronic situations from the perspective of continuity. Develops knowledge related to the methodology of therapeutic education.

Course contents

Chronic illness and disability

Definitions, epidemiology and characteristics of chronicity and disability. The stages of chronicity. Types and new concept of disability. Strategies for quality care of the person with disability. The concept of "Fragility". Testimony of a person with Disability

Therapeutic Patient Education (TPE):

The concept, developmental context, goals and outcomes of Therapeutic Patient Education. The 1998 WHO Dossier. The skills of self-care/self-management and adaptation to chronic illness. The stages of ETP: data collection, educational diagnosis, goals/contract, methods and tools, educational interventions, and evaluation.

Measurement scales

The uni- and multidimensional assessment in the elderly. Main scales used for the assessment and monitoring of chronic and disability conditions and for the assessment of quality of life: ICF, life activities and instrumental life activities, BINA scale, comorbidity index, Mini Mental State Examination, Geriatric Depression Scale, Global Deterioration Scale etc.

Chronicity Clinic

Main Nursing Diagnoses/Welfare Problems to be used in chronic diseases: Acute Mental Confusion, Wandering, Fatigue, Chronic Pain etc. Other Problems: Arthrosis, Osteoporosis, Falls

Rehabilitation in chronic conditions.

Definition and goals of rehabilitation. Types of rehabilitation programs. Main rehabilitative care problems ( urinary and fecal incontinence, mobility). Care plan for the myelopathic patient. Care Plans in Multiple Sclerosis, Chron's Disease and Amyotrophic Lateral Sclerosis.

The Elderly.

Demographic aspects at world, European, Italian and Emilia-Romagna region levels. Care needs in the elderly. Assisting the elderly with impaired function: cognitive decline and dementia, depression, polypathologies, malnutrition, dehydration. (specific care plans). Alzheimer's disease and Parkinson's disease. Exercise programs for the elderly. Vaccinations in the elderly. Legal issues and nonpharmacological therapies in dementia.

Organizational models and care pathways in chronicity.

Principles of public-private collaboration and social-health integration. Facilities for the elderly and organizational-functional and structural characteristics: Day Care Center, Housing Community, Retirement Home, Protected Home. Family House and its characteristics. PNRR and Decree No. 77 of May 23, 2022 "Regulation defining models and standards for the development of territorial care in the National Health Service." Protected Discharge and the Individual Care Plan. Geriatric assessment unit and nurse practitioner. Care pathways and PDTA continuity. Organizational models in chronic care: Chronic Care Model. The Community Hospital and nursing clinics. National Chronicity Plan.

Care giver and Educational Programs.

Care-Giver involvement and education in the care of the complex patient at home. The Young Care-giver Examples of Educational Programs: the PEG Patient, the Hypertensive Patient, the Patient Taking Coumadin

It will be decided with the student group whether to cover some topics in less detail

Teaching methods

Interactive face-to-face lectures. Mid-course test for learning.

A time for interactivity is planned: Roundtable discussion with a totally blind disabled person about living with the disease and acquired disability and his or her relationship with health care providers (about 1 hour time)

Students can ask questions and interact along with the lecturer present

Assessment methods

Tests for learning are expected in the middle of the course. Thereafter, the examination will be written, a series of multiple-choice tests.

Teaching tools

The lecture materials used will be made available by the lecturer online prior to the lecture itself. Further study material will be indicated. It will be specified which material is necessary to study in order to take the exam
