Learning outcomes

At the end of the course, the student must know: the regulatory guidelines and the strategies on which the organization of the health and social-health supply is based, capable of responding to the epidemiological, demographic and social transitions that characterize the demand for assistance; the models of primary care, intermediate care and tha network of health and social-health services that guarantee integration and continuity of care; the main models of taking charge in chronicity; the principles of co_planning and community empowerment.

  • Key concepts: description and epidemiology of conditions FRAGILITY,CHRONICITY, COMORBILITY, DISABILITY, NOT SELF-SUFFICIENCY
  • New healthcare paradigms: initiative healthcare vs at-at healthcare, Chronic Care Model as the backbone of the National Chronicity Plan
  • The professionals promoting the taking charge of chronicity: nurses and the role of Care\Case\Disease Manager in the ambit of new territorial and home care pathways, for taking charge and health management
  • Description of the concepts of integration of care: the coordination of continuity of care with the model of transitional care
  • PNRR and Decree of 23 May 2022, n.77 "Regulation containing the definition of models and standards for the development of territorial assistance in the National Health Service"
  • Tools for the multidimensional assessment of the elderly and patients with chronic pathologies
  • The construction of the protected discharge pathways of the Romagna Local Health Authority: activities of the NuCOT-Bad Management service

Assessment methods

Oral test in small groups

The purpose of the oral test is to verify the student's ability to apply their own knowledge and make the necessary logicla deductive connections inherent to a clinical case.

Gradation of the final grade: analytical ability which emerges only with the help of the teacher, overall correct expression in language= 18-19; independent analysis ability, expression in correct language= 20-24; ability to carry out critical analysis, master specific terminology= 25-29; ability to carry out analysis and connection, full mastery of specific terminology and argumentation and self-reflection= 30-30L
