Learning outcomes

At the end of the module the student has the knowledge of methodology, organizational and clinical care models for the management, management and continuity of care in chronic situations. He develops knowledge of therapeutic education. Knows family and community resources.

- Key concepts: description and epidemiology of conditions of FRAGILITY, CHRONICITY, COMORBIDITY, DISABILITY, NOT SELF-SUFFICIENCY

- New healthcare paradigms: initiative healthcare vs waiting healthcare

- Chronic Care model as the backbone of the National Chronicity Plan

- Description of the concepts of integration of care

- Hospital and Territory integration to ensure continuity of care

- The professionals promoting the management of chronicity

- Chronic diseases and their characteristics

- Tool for the multimensional evaluation of the elderly and of patients with chronic diseases

- The interprofessional Rehabilitation Team

- Rehabilitation nursing and fields of application; conceptual model of Nancy Roper

- Disability treatment and conpensation. Prosthesis, orthoses and aids
