Learning outcomes
At the end of the module, the student possesses the knowledge relating to the methodology, organizational and clinical assistance models for taking charge, management and continuity of assistance in chronic situations. Develop knowledge related to therapeutic education. Knows the resources of the family and the community.
Course contents
1. Principles of taking charge of the person with chronic disease
a) demographic aspects
b) concepts of chronicity, frailty, comorbidity, disability, non self-sufficiency
c) healthy aging and disease trajectory
d) caregiver
2. Models of management and care settings in chronicity
a) transitional care, continuity of care, planned discharge, home assistance
b) individual and family self-management theory
3. Chronic Nursing Care
a) hypertension
b) heart failure
c) liver cirrhosis
d) Alzheimer's dementia
e) Parkinson's disease
f) thyroid diseases
g) rheumatological diseases

Teaching methods
Frontal and interactive lecture with small group exercises and analysis of clinical cases.
Assessment methods
Single Written Exam in the Medical and Chronic Areas + Oral Exam.
Single Written Exam: Care planning for a clinical case in the medical or chronic area, identifying two DIs and one PC (or one DI and two PCs), complete with related factors, defining characteristics, objectives, indicators, and interventions (preparatory to the oral exam) [30 minutes].
Oral Exam: Open-ended questions on the medical area program.
The written exam is passed if all three clinical assessments are complete and correct.
