Learning outcomes

The student, upon completion of the module, knows the clinical and organizational strategies, activities, and tools to identify and reduce the risk of harm to caregivers, team, and visitors and ensure safe care.

Course contents

Fundamentals of Patient Safety and Clinical Governance

Quality and safety of care; principles of Clinical Governance; safety culture and continuous improvement.

Errors and Risk in Healthcare Systems

Healthcare errors; human, professional, organizational, and systemic factors; negligence, recklessness, and lack of professional competence; adverse events, no-harm incidents, near misses, and sentinel events.

Risk Management and Clinical Governance Tools

Risk identification, assessment, treatment, and monitoring; guidelines; protocols; procedures; good clinical and care practices; Ministry of Health recommendations; clinical audit.

Reporting Systems and Learning from Errors

Incident Reporting; SegnalER; reporting and management of adverse events, near misses, and sentinel events; reporting pathways; documentation and traceability of care; learning from errors.

Clinical Risk Analysis Methodologies

Reactive and proactive approaches; Root Cause Analysis (RCA); the 5 Whys technique; Ishikawa diagram; flow chart; Failure Mode and Effects Analysis (FMEA); Risk Priority Number (RPN); corrective actions and monitoring.

Human Factors and Non-Technical Skills for Safety

Situational awareness; decision-making; effective and structured communication; SBAR; teamwork; leadership; management of stress, fatigue, and workload.

Just Culture, Nursing Debriefing, and Organizational LearningProfessional Liability and Medico-Legal Aspects

Professional nursing liability; patient safety; Law No. 24/2017 (Gelli-Bianco Law); civil and criminal liability; ethical and professional principles; medico-legal value of healthcare documentation.
