Learning outcomes

Upon completion of the module, the student is able to know methods and tools for conducting a basic nursing assessment.

Course contents

Epistemological and Methodological Foundations of Nursing Assessment

Nursing process: definition, purposes, and structure; assessment as the foundational phase of the nursing care process.

Methodology of Nursing Assessment

Purpose and process of assessment; initial/comprehensive assessment; focused/targeted assessment; urgent/emergency assessment; reassessment/follow-up; systematic collection, organization, and interpretation of data; identification of care needs, resources, and risk factors.

Conceptual Framework for Assessment: Gordon’s Functional Health Patterns

Theoretical and conceptual foundations; the eleven Functional Health Patterns; functional and dysfunctional patterns; a multidimensional approach to the person; organization and systematization of assessment data.

Methods and Techniques for Collecting Assessment Data

Subjective and objective data; primary and secondary sources; clinical and nursing observation; interview and nursing history; therapeutic communication and active listening; HELP; SOLER; physical examination: inspection, palpation, percussion, and auscultation.

Validation, Organization, and Documentation of Assessment Data

Data validation; organization and systematization of information; nursing documentation; nursing records; quality, accuracy, and appropriateness of documentation.

Ethical, Deontological, and Legal-Professional Dimensions of Assessment

Professional and documentation-related responsibility; deontological principles applied to assessment; Code of Ethics of the Nursing Professions: Articles 3, 17, 20, and 29; confidentiality and professional secrecy; authenticity, accuracy, and truthfulness of documentation; material falsification and ideological falsehood.

Standardized Tools for Nursing Assessment

Principles of use, scoring, and interpretation; functional assessment: Barthel Index; pain assessment: NRS, PAINAD; risk of pressure injuries: Braden Scale, Norton Scale; neurological assessment: Glasgow Coma Scale (GCS); fall risk: multifactorial assessment; nutritional assessment: Mini Nutritional Assessment–Short Form (MNA-SF); cognitive assessment and delirium: Confusion Assessment Method (CAM).
