Learning outcomes
At the end of the module, students will have acquired knowledge of the methodology, organizational models, and clinical care models for taking charge of patients/families/communities in order to plan and manage care in chronic situations with a view to continuity. They will develop knowledge of therapeutic education methodology.

1. Principles of caring for people with chronic diseases
a) demographic aspects
b) concepts of chronicity, frailty, comorbidity, disability, and dependence
c) healthy aging and disease trajectories
d) caregivers
2. Models of care and care settings in chronic conditions
a) transitional care, continuity of care, planned discharge, home care
b) theory of individual and family self-management
3. Nursing care planning in chronic conditions
a) hypertension
b) heart failure
c) liver cirrhosis
d) Alzheimer's dementia
e) Parkinson's disease
f) thyroid disorders
g) rheumatological diseases
