Learning outcomes

At the end of the module, the student possesses the knowledge relating to the methodology, organizational and clinical assistance models for taking charge, management and continuity of assistance in chronic situations. Develop knowledge related to therapeutic education. Knows the resources of the family and the community.

Course contents

1. Principles of taking charge of the person with chronic disease
a) demographic aspects
b) concepts of chronicity, frailty, comorbidity, disability, non self-sufficiency
c) healthy aging and disease trajectory
d) caregiver


2. Models of management and care settings of the person with chronic disease
a) transitional care, continuity of care, planned discharge
b) intermediate care, home assistance, long-term care and integration
c) individual and family self-management theory and self-care theory


3. Nursing care planning for the patient with chronic disease
a) hypertension
b) heart failure
c) arrhythmias
d) hydroelectrolytic disorders: volume depletion, fluid overload, hypo-hypernatremia, hypo-hyperkalaemia
e) chronic kidney disease
f) cirrhosis of the liver
g) dementia
h) Parkinson's disease
i) hyperthyroidism and hypothyroidism
l) rheumatic diseases.
