Learning outcomes

At the end of the module the student: - is capable to manage pharmacological therapies in chronic patients; - is capable to monitor the therapeutic adherence and to provide recommendations on the taking of drugs, their interactions and interactions between drugs and foods; - knows how to put the patient at the center of pharmacotherapeutic interventions in order to achieve better therapeutic adherence.

Course contents

Definitions of pharmaceutical care; prevention, detection and resolution of medication-related problems; patient-centred care approach; patient counselling; pharmaceutical care and treatment adherence; integrated care: interprofessional communication in pharmaceutical care. Medication review. Drug-drug interactions; drug-food interactions; drug-herbal drugs interactions. Documenting pharmaceutical care; outcomes of pharmaceutical care. Pharmaceutical care, health promotion, and disease prevention. Pharmaceutical care and dispensing medicines; OTC medication and pharmaceutical care. Gender and health; support for complex conditions and for vulnerable people. Pharmaceutical care in the aged. Pharmaceutical care in paediatrics. Pharmaceutical care in pulmonary diseases. Pharmaceutical care in type-2 diabetes. pharmaceutical care and cardiovascular diseases/anticoagulation therapy. Pharmaceutical care in viral diseases. Pharmaceutical care for cancer outpatients.
