Transitions in the 20th century and the demographic challenge.

The role of Primary Health Care (PHC) as a response to demographic and epidemiological transition

Patient-centered care models and patient health and care needs

Integration between hospital and territory and between health and social care as a response to the challenge of future healthiness.

The "network" as a possible declination of the request for integration of health services

Reflections on the sustainability of the health system of the future
